00 · Preface

This guide walks through the practical steps of running a consultation through TaloeMed — from signing in, to recording, to reviewing the AI-generated SOAP note, to exporting it for your records. TaloeMed is a Clinical Decision Support System: every AI output is advisory, and the treating physician remains the final decision-maker on diagnosis and treatment.

Signing in

Google Sign-In on iOS and web. Sessions persist on the device.

TaloeMed runs as a native iOS app and a web app at app.taloemed.com. Sign in with the Google account you registered with. Sessions persist on the device; you do not need to sign in again for each consultation.

Google sign-in is used to verify the registered practitioner account; TaloeMed does not request Calendar, Contacts, or Drive access.

If you cannot sign in, check that your account is provisioned in the registered-doctor allowlist. New practitioners should contact support before their first consultation.

Setting up your profile

Fields from Settings → Profile drive what appears on exported documents.

On first sign-in, open Settings → Profile to confirm:

  • Full name (used on exported SOAP notes and prescriptions)
  • Specialty (drives the reasoning context the AI applies)
  • Clinic name and address (appears on exported documents)
  • Medical registration number (used on exports for clinical legitimacy)
  • Consultation timings (used by your patient-facing booking flow if you opt in later)

Settings → Clinic and Settings → Specialty are editable any time. Changes apply to the next consultation; documents already generated are not retroactively updated.

Registering a patient

Patients must be registered before recording begins; identifiers assign automatically.

A patient must be registered before recording begins. Tap the new-patient affordance on the dashboard and enter:

  • Full name
  • Phone number (used for follow-up reminders if you enable them)
  • Age and sex (used by the AI for age- and sex-appropriate differential diagnosis)

A patient identifier (UHID or OP number, depending on your clinic settings) is assigned automatically. The same patient can be selected for follow-up visits without re-entering demographics.

Patient consent

Before recording, you are asked to capture the patient's consent. The consent screen presents the purpose plainly: audio is recorded, transcribed, and used to generate a clinical note that you will review and edit. The patient may withdraw consent at any time.

Consent capture is a requirement under the Digital Personal Data Protection Act 2023 (DPDP Act). The app records when consent was obtained alongside the session so that the audit trail is complete.

ABDM linkage (linking the consultation to the patient's ABHA health identifier) is listed in Settings as Sandbox approval pending — this flow is not yet active in production.

Recording the consultation

Tap Record. Speak in Telugu, English, or a mix. Audio is retained for 30 days.

Tap Recordon the patient's screen. The mic indicator confirms capture. Speak naturally in Telugu, English, or a mix — the transcription pipeline is tuned for Indian-clinic audio.

You can pause and resume within a session. Tap Stop when the consultation ends. Audio is uploaded over an encrypted connection and queued for transcription. Network drops are handled gracefully: the recording persists on the device until upload succeeds.

Audio recordings are retained for 30 days for quality review and then automatically deleted. The transcribed text and generated SOAP note are retained for as long as the session is kept on your account.

Reviewing the SOAP note

Four structured panels — Subjective, Objective, Assessment, Plan — each with a focused editor.

When the SOAP note is ready, tap into the session from the dashboard or Patient Hub. The note is structured into four sections:

  • Subjective: chief complaint, history of present illness, past medical history, social history.
  • Objective: vitals (if dictated), examination findings, observations.
  • Assessment: primary impression, differential considerations, clinical reasoning.
  • Plan: medications (structured: drug, dosage, frequency, duration, route, instructions), investigations, procedures, referrals, patient education.

Each section has an Edit button that opens a focused editor. Make corrections, then save. Each save records the correction so that future model improvements learn from your edits.

Behind the Assessment panel sits a transparent reasoning trace: a clinical-completeness check, a Bayesian differential ranking, an adversarial self-critique, per-axis confidence inputs, an error-taxonomy tag, and an outcome decision. Each stage runs in order on every SOAP draft.

Reading the TrustGate score

Twelve quality gates aggregate to a score out of 100. Lower scores prompt closer review.

TaloeMed runs 12 clinical quality gates against every generated SOAP note. The aggregate score (out of 100) appears on the SOAP screen as TrustGate. The score signals how well the AI-generated draft satisfies the structural and clinical checks the gates apply — not a measure of clinical correctness.

A high score does not replace clinical review. A lower score is a prompt to read the note more carefully before exporting. After you save an edit on a section, that section carries a corrected marker; the TrustGate score reflects the original draft and is not re-computed in real time.

Exporting and sharing

Four formats — PDF, DOCX, prescription, after-visit summary — via the iOS share sheet.

From the SOAP screen, tap Sign & Send to export. Formats include:

  • PDF: the standard clinical note for filing, sharing with the patient, or printing.
  • DOCX: editable Word document if you need to combine notes with other clinic templates.
  • Prescription (Rx): medication-focused export sized for the patient to carry to a pharmacy.
  • After-Visit Summary (AVS): patient-facing handout in plain language, with the prescription and follow-up advice.

Exported files open in the iOS share sheet. From there you can save, AirDrop, email, or send via WhatsApp. Patient identifiers on the export reflect what you entered during patient registration — verify them before sharing.

Bulk export of multiple sessions as a ZIP is available from Settings → Subscription → Data export. The ZIP includes a MANIFEST.csv with patient name and identifier columns so large archives are scannable.

Offline and local-save behaviour

Recording continues offline; SOAP generation needs a live connection; exports work fully offline once persisted.

TaloeMed is designed for clinics with variable connectivity. The app records audio to local storage first and uploads it when the network is available. If you lose connection mid-session, recording continues and the upload queues for later.

SOAP generation requires a live connection to the AI pipeline — once the audio is uploaded, generation typically completes within a minute. If generation fails for network reasons, the session is preserved and retry is offered from the SOAP screen.

Exports and sharing work fully offline once the SOAP note has been generated and persisted to the device.

Privacy basics

Built around DPDP Act 2023. Data hosted in India; audio auto-deleted after 30 days.

TaloeMed is built around DPDP Act 2023 requirements. The short summary:

  • Patient data is never sold, rented, or used for advertising.
  • Personally identifying information is redacted before audio or transcripts are sent to third-party AI providers for processing.
  • Audio recordings are encrypted in transit and at rest, and auto-deleted after 30 days.
  • Consent is captured per consultation and is withdrawable; the audit trail is visible to you in Settings → Consent history.
  • Account data is hosted in India (Mumbai region).

Troubleshooting

Sign-in fails: verify your Google account matches the one registered with TaloeMed. If you recently changed it, contact support.

Recording stops unexpectedly: check that microphone permission is granted (Settings → TaloeMed → Microphone on iOS). If you take a phone call during recording, recording pauses; resume manually after the call ends.

SOAP note did not generate: open the session from the dashboard and tap retry on the SOAP screen. If retry fails repeatedly, check connectivity and try again when the network is stable.

Export shows wrong patient details: open Patient Hub, correct the demographics, and re-export. The export reads live patient data, not a snapshot at recording time.
